Failure effect severity
It should be noted that every conceivable failure effect must be allocated a separate S value. The effects for the relevant customer perspectives should also be developed with a separate structure. Typical customer perspectives include:
1. The client: The supplier’s company – typical effect: Additional work in development, rejects in manufacturing;
2. The client: “system integrator and assembly plant” – typical effects: Rejection in incoming goods or complaint from assembly
3. The client: “system users" – typical effects: Breakdowns, limited functionalities combined with field failures
4. The client: “society” – typical effects: Nonobservance of normative, official, or legal specifications
